Enrollment Requirements
The State of North Carolina requires these by the first day of school (immunizations by 30 days after school starts), but if possible, bring them to your Kindergarten Screening appointment.
YOUR CHILD MUST ATTEND THE FIRST DAY OF SCHOOL to keep his or her slot.
To complete the enrollment process, we will need:
Proof of North Carolina Residency
This can be a utility statement, lease agreement, mortgage statement, guardianship documents, etc. THIS IS NEEDED EACH YEAR, FOR ALL STUDENTS.
Kindergarten Health Assessment Form
We will need Kindergarten Health Assessments filled out by families and the child's pediatrician. Download Kindergarten Health Assessment Form (PDF) (be patient, it's a slow download).
Immunizations
North Carolina state law requires parents/guardians to provide proof of immunization (or a written request for a waiver) within 30 days of a student's first day of school.
Parents who do not wish to have their child immunized based on religious beliefs, or advice from their doctor that immunization poses a risk to their student's health, must submit their request for a waiver in writing, including their physician's statement.
Immunization Records must be attached to the Kindergarten Health Assessment.The following immunizations are required:
- DPT (diphtheria, tetanus, and pertussis): Five doses. If the fourth dose (booster) is given on or after the fourth birthday, the fifth dose is not required.
- Oral Polio Vaccine (OPV): Four doses, one of which must be a booster given on or after the fourth birthday. If the third dose is given on or after the fourth birthday, the fourth dose is not required.
- MMR (measles, mumps, rubella): Two doses, with the first dose given on or after the first birthday and a second dose given before entering school.
- Haemophilus influenza, b (HbOC, or PRP-OMP): At least one dose, given on or after the first birthday and before five years of age. (Second dose is not required if the first dose was given after 15 months of age.)
- Hepatitis B: Three doses.
- Varicella (chicken pox): One dose on or after age 12 months and before age 19 months (not required if born before April 2001).
